In participating in Fiddle Pal Camp and its activities, including those off site, I acknowledge that I, as participant, parent or guest, understand there are risks of accidents and illness. I, as participant or parent/guest do I hereby release and discharge, indemnify and hold harmless to the fullest extent permitted by law, any and all liability, claims, demands, costs and expenses, and causes of action whatsoever which I might have against Fiddle Pal Camp, Fiddle Pal Music LLC, St Paul Lutheran Church, Trinity Lutheran Church, and any of its employees, volunteers, owners and officers. I give my permission for professional medical treatment, transportation to and treatment at a hospital if necessary. I agree to be the party responsible for all medical expenses incurred. I understand that Fiddle Pal Camp cannot guarantee food offered will be free of all products included but not limited to nuts, wheat, soy, dairy and other allergens. I understand the refund policy. I give permission for the use of my, or my child’s image, photo or video for, but not limited to, camp promotion. I acknowledge that I understand the content of this document. I am aware that it is legally binding and I sign it out of my own free will.
